- want to register at the surgery
- live within our catchment area
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
- your NHS number
- details of previous GP surgery
- basic health and medical information
If you will not be in the area for more than 3 months, you will need to complete a temporary resident form.
