Registering with the surgery
To register with the surgery:
- fill out a new patient registration form online
You will be asked for proof of address when registering with the practice. This is to ensure that the information provided will ensure that any NHS service you require will be registered at this address. It also allows us to confirm/ verify this information with our registration department if requested.
